The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options

For your convenience, we accept cash, personal checks, debit cards, apple pay and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Dental Insurance

We accept most PPO insurance plans. Your insurance information is collected at the time you make an appointment. Our team will verify your coverage, where possible. Some companies require a confirmed care plan before verification is possible.

If you need additional service outside of our offices, such as a periodontist, we will coordinate your care, and plan the case together to ensure you get the results you want.

Our team will review the costs with you and set up payment accordingly. We accept all credit cards, CareCredit®, and HSA plans for your convenience.
